Description:
The responsibility of the Community Lifestyles Coordinator is to provide service coordination for all services. This person would be responsible for providing day-to-day leadership and management for the delivery of service program(s), housing and property maintenance, and staffing within the Pepper Pike community. Ensure that all policies, procedures, certifications, and paperwork are kept current and in compliance with all applicable organizations (such as the Ohio Department of Developmental Disabilities, County Board, HUD, and the Department of Labor). Represent MCA and all Affiliated entities professionally and positively in the community, forming advantageous community relationships and partnerships.
Duties Include:
a. Make a connection with the families and provide needed support as they arise
b. Assist the families with obtaining services as needed
c. Identify and meet the needs of the community
d. Manage all new and existing provider intake documentation, service placement, and ongoing changes or updates
e. Schedule and participate in REP (Resident Enrichment Program) activities with the individuals
f. Be involved in the community and develop relationships and partnerships to enhance our programs
g. Coordinate with Services Manager, Transportation Manager, and Housing Specialists regarding services for all new and existing consumers within the Pepper Pike community
h. Work with Services to develop and implement Achievement Outcomes for all Pepper Pike consumers which are relevant to the ISPs and services provided
i. Provide training to all relevant staff on new consumers’ ISPs
j. Communicate with County Board of DD personnel (ongoing, as needed) professionally and positively and attend ISP meetings as needed
k. Ensure all health and safety guidelines are followed for all individuals being served
l. Participate, as needed, in the hiring process of new DSPs, including onboarding of new direct care staff to ensure training is complete and necessary paperwork is obtained
m. Maintain open communication with all the HPC, Vocational Habilitation, and Transportation Managers/Staff
n. Ensure consumer schedules are created and updated as needed
o. Handle any consumer issues and crises promptly, including Unusual Incidents
p. Understand and implement the UI/MUI reporting system; receive any daily incidents from Services, Maintenance, and Transportation and report according to reporting policy
q. Address all Unusual Incidents that have been deemed a Major Unusual Incident by the County Board of DD by assisting with all investigations and prevention plans
r. Assist the maintenance department with obtaining quotes, and estimates for properties as requested
s. Oversee and ensure documentation is completed for all services and sites at Pepper Pike
t. Keep current with the DODD regulations by attending seminars and/or training as available with the approval of COO
u. Understand and comply with all MCA policies and procedures
v. Work with HPC, Transportation, and Maintenance supervisory staff to provide training, support, and guidance and to discuss concerns, progress, or areas of improvement, being a role model to them
w. Maintain an open line of communication with HR and the COO
x. Ensure time sheets for staff and consumers are reviewed and approved
y. Participates in the Emergency on-call rotation
z. Complete all duties assigned by the CEO and the COO
MCA Employee Requirements:
a. B.A. or B.S, in Business, Special Education or Social Work
b. MCA has a basic standard for all requirements of the job.
c. Must have at least 5 years of experience working in the DD field.
d. Must be detail-oriented.
e. Must know the rules and regulations of DODD.

Required Skills:
B.A. or B.S, in Business, Special Education or Social Work

Hours:
A minimum 40-hour work week, Monday – Friday.
Wage:
$50,000-$55,000/per year
